Hi Peter,
Your Utricularia graminifo appears to have been left floating for sometime.
Pick a point and gently separate the ball to flatten it out, then further separate it, depending on the size into smaller pieces and plant green side up!

When starting the plant lower light is a good idea, in fact it likes shade, though once it gets established that does not seem as important.
Utricularia graminifo tends to prefer softer water though the plants propagated here are not nearly so sensitive.
Good CO2 levels are a very good idea.
Remember this is a carnivorous plant…
