Can't tell you it's one exact thing or another. Rotala rotundifolia varieties are fast growing and will be one of the first 'warning signs'. APT is rather low concentration so more frequent water changes coupled with increased fertilization are likely a wide strike that helps fix a wide range of issues. Your substrate will get depleted in some nutrients and likely enriched in others. It might be the case that it covered for a deficient fertilization for a while but now it's starting to run out.

I don't know the exact Ca and Mg targets, but with a GH of 8 °dH more remineralizers will get you in a decent range.
